SLATE MEMORIAL PLAQUE
The Legacy Set includes one personalized natural Slate Memorial Plaque and one handcrafted wooden display easel.
The plaque may include your loved one’s photograph, name, dates, memorial passage, poem, verse, prayer, quotation, or other meaningful wording.
Natural slate varies slightly in edge shape, surface texture, thickness, and engraving response. These variations are part of the character of genuine slate and make each plaque unique.
Additional identical Slate Memorial Plaques may be added for $49.99 each. Additional identical plaques use the same approved engraving and personalization and do not include another wooden easel.
MEMORIAL CARD & DOG TAG OPTIONS
The included Memorial Card begins as a Standard Single-Sided wallet card. Available configurations include:
• Standard Single-Sided
• Special Single-Sided
• Standard Double-Sided
• Special Double-Sided
Special Single-Sided formats include Hanging Memorial Card, Fridge Memorial Magnet, and Adhesive-Backed Memorial Card.
The Special Double-Sided configuration is a Double-Sided Hanging Memorial Card.
The included Dog Tag Memorial begins as Single-Sided and may be upgraded to Double-Sided.
Both the Memorial Card and Dog Tag may use Photo Memorial, Poem / Verse Memorial, or QR Code Memorial designs.
Double-sided keepsakes allow the front and back designs to be selected independently.
MEMORIAL VIDEO & QR ACCESS
The included personalized Memorial Video uses up to 30 submitted photographs and is designed as an approximately 2–4 minute digital tribute.
You may provide memories, stories, preferred tone, memorial wording, music preferences, and other guidance to help EverCherish create a tribute that reflects your loved one’s life.
The standard video package includes:
• Up to 30 photos
• Approximately 2–4 minute finished tribute
• 2 revision rounds
• Digital delivery
• EverCherish-managed QR access
A 31–50 photo package is available for an additional $39.99.
Additional revision rounds beyond the two included rounds are available for $25 each.
If a QR Code Memorial design is selected for the Memorial Card or Dog Tag, that QR Code can connect directly to the Memorial Video included with this Legacy Set through EverCherish-managed QR access.
PHOTOS, WORDING & PERSONALIZATION
Clear, high-resolution original photographs generally provide the strongest results for engraving and Memorial Video presentation.
Black typically provides the strongest photograph contrast on engraved Memorial Cards and Dog Tags. Other available colors may produce a softer engraved appearance.
Longer poems, passages, prayers, quotations, or memorial messages may require smaller text, tighter spacing, or layout adjustments.
EverCherish may make reasonable image, spacing, and layout adjustments when preparing each personalized component.
Instructions for submitting Memorial Video photographs and digital materials will be provided after ordering.
ADDITIONAL COPIES, PROOFS & REVISIONS
Additional identical keepsakes may be added during personalization:

Identical copies use the same approved design, personalization, photographs, wording, colors, formats, and QR connection where applicable.
A digital proof will be provided before production. Please carefully review names, dates, photographs, wording, layout, colors, QR selections, and other personalization details before approval.
Two Memorial Video revision rounds are included. Additional Memorial Video revision rounds are available for $25 each.

Qualifying EverCherish durable memorial keepsakes are backed by the applicable EverCherish lifetime replacement warranty while EverCherish remains in business.
Each Legacy Set is individually prepared and personalized with care.
Production begins after the required photographs, wording, and other materials have been received and the applicable proofs have been approved. Rush Production may be available for eligible orders.
